Underwear for vulvar lichen sclerosus: what to wear and what to avoid
Underwear in direct contact with the vulvar area can make a real difference to VLS symptoms. These are my recommendations.
How to wash the vulvar area with VLS
100% cotton, untreated. It is the most suitable material for underwear for VLS. It is breathable, non-irritating, and does not retain moisture. Organic cotton without bleaching agents or harsh dyes is even better.
Avoid: synthetics (polyester, nylon, lycra), lace in direct contact with the skin, fabrics treated with fabric softeners or fragranced “hypoallergenic” finishes.
Bamboo is a suitable alternative to cotton — soft, breathable, and naturally hypoallergenic.
The cut matters too
Loose-fitting underwear. Compression in the vulvar area increases local heat, friction, and itching. Tight underwear — especially thong styles or those that compress the labia — worsens symptoms in many patients.
No seams in the vulvar area. Seams in direct contact with the affected area create mechanical friction that can trigger itching or lesions.
Avoid: tight trousers for prolonged periods, cycling or running clothing without a cotton lining, compression tights without a cotton gusset.
Small habits with a big impact
Wash underwear with fragrance-free detergent. Residual detergent on clothing can irritate vulvar skin. Use fragrance-free detergent and run an extra rinse cycle.
Avoid fabric softener. Fabric softener leaves fragranced residues in the fibres.
At home, go without underwear when possible. This allows the vulvar area to breathe and reduces friction during rest hours. Many patients report reduced nighttime itching when sleeping without underwear.
