Can I swim in a pool or in the sea with vulvar lichen sclerosus?
Yes, although with some precautions. Pool chlorine can irritate sensitive vulvar skin. It is recommended not to stay in the water for too long, to shower with warm water after getting out, to dry the vulvar area thoroughly, and to apply the moisturizing oil after showering. Seawater is usually better tolerated than chlorine.
Can I use sanitary pads with lichen sclerosus?
Fragranced pads or pads with perfumed gel are contraindicated. Fragrance-free cotton pads are the safest option. Many patients with VLS prefer menstrual cups or discs, which avoid contact between synthetic materials and the vulvar area.
Physical activity and everyday life
Can I exercise with vulvar lichen sclerosus?
Yes. Exercise is not contraindicated in VLS. However, some types of exercise may increase friction or heat in the vulvar area. Sportswear with a cotton lining is recommended, as well as showering after exercise with warm water and applying vulvar oil after showering. Contact sports or activities with significant friction may require individual adjustments.
Can I remove hair from the vulvar area with lichen sclerosus?
Hair removal in the vulvar area with VLS requires caution. Hot wax can irritate fragile skin and cause lesions. Shaving can cause microtrauma that worsens inflammation. Laser hair removal requires an individual assessment. If you remove hair, do so with extreme care and assess individual tolerance. Many patients choose to reduce or stop hair removal during the active phase of the disease.
Sexuality and relationships
What lubricant can I use with vulvar lichen sclerosus?
Fragrance-free, glycerin-free lubricants are the most suitable. Glycerin can promote yeast infections on sensitive vulvar skin. Water-based lubricants are compatible with latex condoms; silicone-based lubricants are not compatible with silicone toys but are compatible with condoms. Lubricants with fragrance, parabens, or warming agents should be avoided.
Can I get pregnant with vulvar lichen sclerosus?
VLS does not affect fertility. However, pregnancy may change the course of the disease—some patients report improvement during pregnancy, possibly due to hormonal changes. It is important to inform your obstetrician of the diagnosis and to adjust treatment during pregnancy with your specialist.
Factors that influence the disease
Does stress worsen vulvar lichen sclerosus?
Stress can modulate the immune response and worsen symptoms of many autoimmune diseases, including VLS. Many patients report that flare-ups coincide with periods of intense stress. However, stress does not cause VLS—it may be a modulating factor in patients with a predisposition. Managing stress makes sense as part of overall health care, not as a specific treatment for VLS.
Do sexual relations worsen lichen sclerosus?
Sexual intercourse itself does not worsen the disease, but mechanical friction on inflamed tissue can cause pain and, in some cases, small lesions. With adequate lubrication and control of active disease, most patients can have intercourse without worsening the clinical situation.
